Is there someone you think would benefit from a call from VISIONS?  Fill out a Referral Form and fax forms attn: Rehabilitation department to (212) 219-4078 or email to crivera@visionsvcb.org.

To receive services VISIONS must receive a Medical Eye Report which has been filled out within the last six months.
